Integration of Physical & Behavioral Health

Building bridges between primary care, behavioral health providers, community-based organizations, and social service agencies to implement a whole-person approach to care. 


To better address physical and behavioral health needs through an integrated network of providers. This project will support the Washington State Health Care Authority’s initiative to bring together the financing and delivery of physical and behavioral health services, through Managed Care Organizations.

Addressing Disparity

This project works to better provide for all Medicaid beneficiaries (children and adults) particularly those with or at-risk for behavioral health conditions, including mental illness and/or substance use disorder (SUD).


OCH has aligned with the Practice Facilitator with the Pediatric Transforming Clinical Practices Initiative (P-TCPi) through the Department of Health (DOH) and the Coach Connector with the Practice Transformation Support Hub (PTSH) through Qualis Health and a grant from the DOH.

Partner Success:

Kitsap Recovery Center has brought in trauma informed and 12 step yoga instructors on-site twice a week.
