Natural Communities of Care

A Natural Community of Care (NCC) is a group of partners that serve the same Medicaid population due to geographical proximity, natural referral patterns, and collaborative service agreements. For non-clinical partners, the benefits of participating in NCC convenings are shared learning, networking, and informing the shared commitments of Implementation Partners under the Medicaid Transformation Project (MTP). There are no costs to participating in NCCs.

Each Natural Community of Care has a Shared Change Plan (SCP). The SCP represents an agreement on shared expectations, goals, and strategies of the Medicaid Transformation Project. Partners provided valuable input on SCPs at our NCC convenings.

Regional NCC Convenings

The 2019 regional Natural Community of Care convening took place on June 25 at the Jamestown S’Klallam Tribe Red Cedar Hall. Based on initial review of evaluations by participants, the convening successfully achieved its objectives:

1. To create space for sharing knowledge in alignment with the work and goals of the collective Olympic Community of Health
2. To build and strengthen partnerships across the region
3. To learn about early successes and best practices of partner work
