Port Gamble S'Klallam Tribe's Behavioral Health Integration Team

The Port Gamble S'Klallam Tribe has been working to integrate their medical, dental, and behavioral healthcare for the past year, forming a Behavioral Health Integration (BHI) team in early 2018. This team meets bi-monthly to address staff training needs, design behavioral health screen tools, develop new community partnerships and practice coaching for quality improvement in care coordination, and much more. Under the leadership and vision of Health Director Jolene Sullivan, the Port Gamble S'Klallam Tribe will break ground on a new Community Health Services Building in fall 2019.


The BHI team recently spent the day with the Tulalip Indian Tribe who graciously hosted and shared lessons learned and best practices from their own community health services building and integrated care.


The Port Gamble S'Klallam Tribe is an OCH Implementation Partner and active in the Kitsap Natural Community of Care.
